- Third-quarter revenue rose 3.6 percent, while net income attributable to HCA Healthcare fell 3.9 percent.
- Same-facility equivalent admissions increased 3.0 percent, but uninsured volume weighed on profitability.
- Management expects full-year 2026 results in the upper half of its existing guidance ranges.
HCA Healthcare reported stronger patient demand but a significant payer-mix burden in the third quarter ended September 30, 2026. The company said in its October 27 results release that revenue increased 3.6 percent year over year to USD 19.849 billion.
Growth offsets a costly coverage shift
Net income attributable to HCA Healthcare declined 3.9 percent to USD 1.579 billion. Adjusted earnings per diluted share, a non-GAAP measure, increased 3.7 percent to USD 7.22, while adjusted EBITDA rose 1.2 percent to USD 3.918 billion.
Operating volume remained a bright spot. Same-facility equivalent admissions increased 3.0 percent, and emergency-room visits rose 5.1 percent. At the same time, same-facility inpatient surgeries fell 1.4 percent and outpatient surgeries declined 3.7 percent. Revenue per same-facility equivalent admission increased 0.6 percent.
HCA said patients losing coverage on health insurance exchanges were moving into the uninsured category. The company estimated that this payer-mix shift reduced income before income taxes by approximately USD 400 million in the quarter. The pressure was partly offset by higher insured revenue per admission, improved cost performance and a modest increase in net benefits from Medicaid supplemental payment programs.
Management now expects full-year 2026 operating results to land in the upper half of its previously issued guidance ranges. Those ranges call for revenue of USD 77.0 billion to USD 79.5 billion, net income attributable to HCA Healthcare of USD 6.3 billion to USD 6.7 billion, and adjusted EBITDA of USD 15.4 billion to USD 16.1 billion.
For investors, the result presents a mixed operating picture. HCA is still producing volume growth and higher adjusted earnings per share, but coverage losses are changing the economics of care and weighing on margins. The company also repurchased 7.230 million shares for USD 2.970 billion during the quarter, supporting per-share results while reducing the share count.
