- Third-quarter revenue increased as same-facility equivalent admissions gained 3.0 percent year over year.
- Uninsured patient volume reduced pretax income by an estimated USD 400 million.
- Management expects 2026 operating results in the upper half of its existing guidance ranges.
HCA Healthcare's third-quarter results added a new risk detail for investors on Tuesday, October 27, 2026: a shift toward uninsured patients reduced income before taxes by an estimated USD 400 million. The company nevertheless expects its full-year operating results to land in the upper half of its existing 2026 guidance ranges, according to the Business Wire release.
What changed in the latest quarter
Revenue rose 3.6 percent year over year to USD 19.849 billion for the quarter ended September 30. Net income attributable to HCA Healthcare fell 3.9 percent to USD 1.579 billion, while adjusted diluted earnings per share increased 3.7 percent to USD 7.22.
Patient demand remained constructive. Same-facility equivalent admissions increased 3.0 percent, and emergency-room visits rose 5.1 percent. The payer mix moved in the opposite direction as patients losing health-exchange coverage increasingly entered the system as uninsured. HCA said that shift affected pretax income by approximately USD 400 million during the quarter.
Guidance stays in the upper half
HCA retained its 2026 guidance ranges of USD 77.0 billion to USD 79.5 billion in revenue, USD 6.3 billion to USD 6.7 billion in attributable net income and USD 15.4 billion to USD 16.1 billion in adjusted EBITDA. Adjusted EBITDA reached USD 3.918 billion in the third quarter, up 1.2 percent year over year, but its margin narrowed to 19.7 percent from 20.2 percent.
For investors, the quarter presents a mixed operating picture: volume growth and cost improvements support the outlook, while uninsured admissions and policy-related reimbursement pressure weigh on profitability. HCA also declared a quarterly dividend of USD 0.78 per share payable on December 29, 2026, to holders of record on December 15.
