A clinical waste disposal company has been fined £300,000 after an employee's arm was severed while he tried to clear a blockage in an autoclave at its Swindon facility. Tradebe Healthcare National Ltd was also ordered to pay £18,710 in costs following a Health and Safety Executive (HSE) investigation into the February 2022 incident.

The 42-year-old worker lost his left arm from the elbow down when the machinery activated during the blockage-clearing attempt. Surgeons carried out a six-hour emergency operation to reattach the limb, and the employee has since undergone three further procedures. He continues to live with limited movement in the reattached arm.

HSE finds multiple safety failures

The HSE investigation identified systemic problems at the site, including inadequate machinery isolation that should have prevented the autoclave from operating while the worker was inside. Inspectors also found significant gaps in staff training and a lack of proper supervision during hazardous tasks.

The fine was finalised in late August 2026, bringing to a close legal proceedings centred on the company's breach of its duty of care. Tradebe Healthcare National, which specialises in clinical waste disposal, was held accountable for the safety failures that led to the life-changing injury.

Waste sector under increased scrutiny

The penalty comes amid a period of heightened regulatory action across the UK waste and environmental services industry. In a separate case concluded on August 20, 2026, the Court of Session in Edinburgh imposed a nine-year ban on Garry Pettigrew, former managing director of Healthcare Environmental Services (HES).

The court found that Pettigrew breached fiduciary duties and entered into contracts while his company was on the brink of insolvency, before its liquidation in 2019. The cases reflect growing legal consequences for firms and executives in the waste management sector, covering both operational safety and corporate governance.
