Novo Nordisk's Wegovy Pill Outruns the Injection, but 2027 Pricing Clouds the Math

Novo Nordisk has given investors two separate dossiers to weigh this week: a third-quarter report that shows its oral Wegovy more than offsetting a shrinking injection business in the US, and a scientific programme in Washington that may — or may not — add weight to the longer-term story. Neither settles the question that matters most, which is whether extra volume can pay for the price cuts the company has already flagged for the start of 2027.

From 1 January 2027, US list prices for Wegovy and Ozempic are set to fall. The announced monthly list price for both products is USD 675, a reduction of roughly 50% for Wegovy and about 35% for Ozempic. Novo has said the moves will weigh on cash flow next year, and it has not yet issued any growth commitment for 2027.

The pill is carrying the US franchise — for now

The quarterly figures published yesterday offer the clearest starting point. At constant exchange rates, US sales of injectable Wegovy fell 13% in the third quarter of 2026. Including the tablet, the US Wegovy business grew 21%. The pill therefore more than compensated for the injection's decline in that period, a positive signal on demand — though it does not yet answer whether the same offset holds once the new prices take effect.

The tablet itself generated DKK 5.39bn in the third quarter of 2026, up 53% on the prior quarter. Group adjusted revenue came in at DKK 79.2bn, with adjusted operating profit of DKK 31.72bn. On a currency-neutral basis, adjusted revenue rose 4% and adjusted operating profit 27%, a performance Novo attributed to GLP-1 volume growth, above all in obesity.

Should investors sell immediately? Or is it worth buying Novo Nordisk?

Management lifted its full-year 2026 outlook on the back of that. Adjusted revenue and adjusted operating profit are now each expected to land between -2% and +2% at constant currencies, against a previous range of -6% to unchanged. The new band still permits either a decline or growth, which is why the guidance reads as a cautious improvement rather than an outright growth pledge.

A crowded November calendar

Novo said on 29 October that it will present 26 abstracts on its obesity portfolio at ObesityWeek in Washington, running 14–17 November, with data on CagriSema and Wegovy among the highlights. The congress is not a regulatory milestone, and the value on offer is incremental: it may sharpen the picture of the product portfolio, but the content of the presentations will determine whether it does.

Further out, the expected FDA decision on CagriSema is due by the end of 2026, with a US launch planned for early 2027 subject to approval. Neither the verdict nor the commercial outcome should be taken as given.

The sell side stays on the fence

Citi raised its price target while keeping a neutral rating, citing limited visibility on pricing for the coming year and beyond. The broker sees tougher competition from 2028, particularly Lilly's Retatrutide, and trimmed its longer-term estimates by as much as 6% — drawing a line between a brighter near-term picture and a more demanding competitive landscape later.

Jyske Bank, by contrast, lifted its growth expectations for the coming years, pointing to the accelerated rollout outside the US and a tailwind from Medicare Bridge coverage in the United States, while calling the valuation fair relative to its forecasts. It also lists pricing pressure, stronger competition and the semaglutide patent expiry as risks.

Several analyst houses reportedly reiterated hold ratings yesterday, with price targets spanning DKK 275 to DKK 350. The guidance raise, in other words, did not translate into more positive recommendations.

Geography as a partial hedge

Novo intends to broaden the pill's footprint. It is already available in eight markets, and the company plans to offer it in more than 20 countries by mid-2027. Additional markets could open up new demand and widen the room to absorb pressure in the US — valuable if growth is not to rest solely on further American prescriptions. The commercial success of those launches, not merely their execution, is what would count.

Novo Nordisk at a turning point? This analysis reveals what investors need to know now.

The bull case therefore requires more than rising pill sales: the obesity business as a whole would need to expand economically despite the headwinds. If progress stays confined to individual products or markets, a durably cheaper valuation becomes harder to justify. The bear case is simultaneous pressure on price and volume momentum — if rivals pick up patients while injections stay weak, the pill would have to offset several drags at once, and strong demand would not automatically convert into revenue or earnings growth.

A new finance chief and a February test

Separately, Novo said yesterday that chief financial officer Karsten Munk Knudsen will leave the company at the end of 2026, to be succeeded by Luigi La Corte, currently CFO of Galderma. The handover bears on financial steering and communication; on its own it says nothing about the operating business and should not be conflated with the product debate.

What will clarify the economics is the 2027 guidance scheduled for February. It must show the net effect Novo expects from pill growth, the price changes and the product mix. Only then can investors judge whether commercial momentum amounts to a durable growth story — or whether the volume gains are simply buying time ahead of the 2027 list-price reset.
