Novo Nordisk's US Share Erosion Prompts Fresh Target Cuts as Pipeline Bets Face Delays

BMO and Deutsche Bank trimmed Novo Nordisk targets as US share fades and Wegovy's Swedish list price fell 20%, offset by EASD pipeline data.

Novo Nordisk's grip on the American diabetes and obesity market is loosening, and the sell side is taking notice. On Monday, BMO Capital trimmed its price target on the Danish drugmaker to $40 from $47, keeping a Market Perform rating on the shares. The analysts pointed to two forces squeezing the company: fading US market share and a punishing year-over-year comparison that is expected to weigh on third-quarter 2026 revenue for the blockbuster Ozempic, given the outsized base set in the same period of 2025.

The stock edged up 0.4% to EUR 34.52 in Monday's session, though it remains down 22% since the start of the year. BMO is not alone in scaling back expectations. On October 7, Deutsche Bank Research cut its target to DKK 225 from DKK 245 while leaving its Sell recommendation in place.

Price Cuts in Europe Add to the Pressure

Competitive intensity in the US is only part of the story. In Europe, Novo Nordisk lowered the list price of its obesity treatment Wegovy in Sweden by 20%, effective October 1, a move the company framed as an effort to ease the financial burden on patients seeking obesity care. On the same date, Swedish regulator TLV broadened reimbursement for Alhemo to cover all patients aged twelve and older living with moderate to severe hemophilia B.

Clinical Data Offer a Counterweight

Against that commercial backdrop, Novo Nordisk used the annual EASD meeting to showcase a slate of next-generation results. A one-year functional MRI study found that the investigational candidate CagriSema alters how the brain responds to tempting, calorie-dense foods. Separate type 2 diabetes studies showed a reduction in dangerous visceral fat surrounding internal organs, with bone health preserved despite substantial weight loss.

Data from the OCTANE trial added another data point: patients switching from injectable GLP-1 therapies to the Wegovy pill lost an average of 4.1% of body weight over a three-month treatment window. A retrospective analysis of 636,525 adults with type 2 diabetes found that patients who increased their semaglutide dose from 1 mg to 2 mg had a statistically significant 6% lower risk of major cardiovascular events than those who switched to tirzepatide.

Should investors sell immediately? Or is it worth buying Novo Nordisk?

The company also bolstered its pipeline through an exclusive licensing deal with Hengrui Pharma for the oral Phase 1 candidate HRS-1596, covering rights outside mainland China, Hong Kong, Macao and Taiwan. The agreement carries a $300 million upfront payment and could reach a total value of up to $2.6 billion.

Buybacks and a Hemophilia Hurdle

Management continues to lean on capital returns to support the share price. Under a twelve-month program launched February 4, 2026, worth up to DKK 15 billion, the company had repurchased 35,379,179 B-shares for DKK 9,939,842,031 as of September 25. Novo Nordisk's market capitalization stands at EUR 150.22 billion.

Not everything is moving on schedule. Roughly two weeks ago, the US Food and Drug Administration extended its review of Denecimig for hemophilia A, citing ongoing remediation work at a manufacturing site. The agency raised no concerns about clinical efficacy or safety. Novo Nordisk still targets a US launch in the first half of 2027 and has maintained its full-year 2026 financial guidance.

Margin Math Clouds the Recovery Case

Citi offered a more cautious read on profitability, lowering its target to DKK 296 from DKK 310 with a Neutral rating. The analysts expect largely flat margins through 2030, arguing that rising research and development spending will cap margin expansion as the company funds its pipeline.

For investors, the question is whether Novo Nordisk can monetize the shift from subcutaneous injections toward oral formulations and combination therapies before competitive pressure erodes margins. If CagriSema and oral GLP-1 receptor agonists deliver a second growth phase — and if patients can move from shots to pills without sacrificing efficacy — the addressable population expands considerably. The preservation of bone density alongside fat reduction could also give CagriSema a favorable safety profile relative to rivals. Should the pipeline materialize on schedule, the company could rebuild its market value and quiet doubts about its innovative edge.

The risks, however, are concrete. Further regulatory delays or disappointing late-stage data would invite another downward revision of growth expectations. The next major catalyst outside the metabolic space remains the planned US introduction of Denecimig in the first half of 2027, subject to approval. Until then, the pace of clinical development is the yardstick by which Novo Nordisk will be measured.
