Novo Nordisk's Pipeline Promise Meets a Sobering Reality Check

Novo Nordisk finds itself at a crossroads that few would have predicted during its blockbuster ascent. The Danish drugmaker's shares changed hands at EUR 34.56 on Monday in European trading, capping a year-to-date decline of 22%. With the stock sitting 37% below its 52-week peak, investors are grappling with an uncomfortable question: is this a temporary lull in an otherwise intact growth story, or the early stages of a structural slowdown?
The pressure has been building on multiple fronts. At its recent capital markets day in London, management outlined long-term ambitions stretching to 2030 and 2035 but conspicuously avoided near-term operational targets. That reticence did not go unnoticed. BMO Capital trimmed its US price target from USD 47 to USD 40 while maintaining a "Market Perform" rating, and Citi cut its target from DKK 310 to DKK 296 with a Neutral stance, citing expectations of largely flat margins through 2030 as rising research budgets eat into profitability.
The Semaglutide Question
At the heart of the debate lies the durability of the semaglutide franchise. Ozempic and Wegovy accounted for roughly 75.5% of adjusted group revenue in the first half of 2026, yet adjusted US sales fell 4% year-on-year in constant currency during the same period. For the full year, Novo Nordisk projects adjusted revenue and operating profit to land anywhere between unchanged and minus 6% in constant-currency terms.
Part of the distortion stems from a statistical quirk. In the third quarter of 2025, a gross-to-net adjustment worth DKK 2 billion lifted quarterly revenue to DKK 30.7 billion, creating a base effect that makes the current quarter's comparison particularly unforgiving. Beyond the accounting noise, however, declining US market share represents a genuine headwind. How effectively the company can sustain demand without resorting to margin-eroding discounts will serve as the key barometer in the quarters ahead.
Clinical Setbacks and Competitive Erosion
The competitive landscape has grown decidedly less comfortable. In the Phase III REDEFINE 4 trial, CagriSema failed to meet the non-inferiority threshold against Eli Lilly's Zepbound for weight reduction. Separately, Ziltivekimab could not significantly reduce major cardiovascular events in the Phase III ZEUS study. These misses undercut the narrative that Novo Nordisk can effortlessly defend its lead against emerging rivals, forcing higher R&D spending while narrowing pricing flexibility.
Should investors sell immediately? Or is it worth buying Novo Nordisk?
Citi responded by lowering its projected average earnings-per-share growth for 2027 through 2030 from 12% to 9%. Should US pricing pressure intensify further, mid-term margins risk slipping below historical peaks.
Where the Optimism Resides
Not all developments point in the same direction. Data presented at the EASD annual meeting offered fresh evidence that CagriSema alters brain responses to tempting, calorie-dense foods, based on a one-year functional MRI study. Additional research in type 2 diabetes indicated reductions in dangerous visceral fat around internal organs, with bone health preserved despite substantial weight loss.
The OCTANE investigation showed patients switching from injectable GLP-1 treatments to the Wegovy tablet lost an average of 4.1% of body weight over three months. Meanwhile, a retrospective analysis of 636,525 adults with type 2 diabetes found that patients escalating semaglutide from 1 mg to 2 mg had a statistically significant 6% lower risk of major cardiovascular events compared with those switching to tirzepatide.
Citi raised its peak sales forecast for oral Wegovy from USD 10 billion to USD 13 billion, reflecting confidence in the tablet format's commercial potential.
Building the Next Generation
Management aims to bring more than five multi-blockbuster medicines to market by 2030 and expand the patient base beyond 60 million. By 2035, risk-adjusted pipeline revenue is targeted to exceed DKK 150 billion. External partnerships form part of that strategy, including a licensing agreement with Nanexa worth up to EUR 1.165 billion and an exclusive deal with Hengrui Pharma for the oral Phase 1 candidate HRS-1596 outside mainland China, Hong Kong, Macao and Taiwan. That arrangement carries a USD 300 million upfront payment and could reach USD 2.6 billion in total value.
Capital returns continue to underpin the equity story. Under a twelve-month buyback program of up to DKK 15 billion, the company had repurchased 35,379,179 B-shares for DKK 9,939,842,031 as of September 25. By October 9, more than 23 million B-shares had been acquired for over DKK 6.7 billion, providing medium-term support to earnings per share. Novo Nordisk's market capitalization stands at EUR 150.22 billion.

Regulatory Hurdles Beyond the Core
Delays outside the metabolic space add another layer of uncertainty. The FDA extended its review of Denecimig for hemophilia A roughly two weeks ago following manufacturing site corrections. While the agency identified no deficiencies in clinical efficacy or safety, and Novo Nordisk reaffirmed its 2026 financial outlook with a targeted US launch in the first half of 2027, the episode illustrates how regulatory and production risks can disrupt timelines.
The November Test
The next fundamental milestone arrives on November 4, when Novo Nordisk reports third-quarter 2026 results. Citi anticipates group revenue of DKK 73.5 billion and Wegovy tablet sales of DKK 5.3 billion. That release will clarify whether the US slowdown is purely technical in nature or whether the company is genuinely losing momentum in the global obesity market.
Until then, the stock's trajectory hinges on whether management can stabilize US market share and keep margins in line with industry norms. If semaglutide's relative decline persists, further analyst downgrades may follow. Should clinical progress on oral formulations and combination therapies continue, the structural foundation remains intact—but additional regulatory delays or disappointing late-stage data could trigger another round of downward revisions to growth expectations.
