Medtronic is stacking up regulatory wins across its cardiovascular franchise while simultaneously steering the separation of its diabetes unit toward a decisive mid-October cutoff. The twin storylines — one clinical, one structural — are pulling investor attention in opposite directions.
On the clinical front, the medtech giant confirmed FDA breakthrough device designation for its IN.PACT BTK drug-coated balloon catheter, a device aimed at treating vascular disease below the knee. A randomized pivotal trial enrolling roughly 400 patients is planned to build the evidence base needed for eventual U.S. market approval. The FDA's breakthrough program is designed to speed development of technologies addressing serious conditions, giving Medtronic closer access to regulators and priority review pathways — though it offers no guarantee of eventual clearance.
That designation arrived alongside a broader wave of product clearances. Medtronic disclosed on September 30 that its Affera Prism-2 mapping software had secured FDA approval and, at the same time, won CE marking for the European market. European regulators also granted CE certification to the PulseSelect ProxBox adapter and its proximity-indication software. Both tools are built for cardiac ablation procedures, deepening the company's footprint in electrophysiology — a field where regulatory sign-offs are the price of entry for placing next-generation systems in heart centers.
Reimbursement Codes and Clinical Data Round Out the Picture
Beyond device approvals, the American Medical Association has upgraded billing codes for Medtronic's Altaviva and Symplicity Spyral systems from Category III to Category I. The reclassification covers procedures targeting urge incontinence and hypertension and takes effect in January 2028. Medtronic was careful to note that Category I status can inform future reimbursement decisions but does not automatically guarantee payer coverage.

The company filed a Form 425 with the U.S. Securities and Exchange Commission detailing an exchange offer of up to 225,361,295 MiniMed shares for Medtronic common stock. The offer is capped at an exchange ratio of no more than 4.5939 MiniMed shares per Medtronic share.
According to a report from Barron's, the offer has lost some of its appeal after MiniMed shares came under pressure, raising the possibility that shareholders may hesitate to tender — a scenario that could slow the separation process. The exchange window is set to close on October 9 unless it is extended or terminated early, with the final ratio to be determined by volume-weighted average prices from October 5 through 7.
Market Reaction Stays Muted
Despite the flurry of announcements, Medtronic's stock has been largely range-bound. Shares were trading at EUR 77.84 on the day of the latest disclosures, down 0.8%, after closing at EUR 78.44 on Monday. That Monday session had seen a 2.2% gain, lifting the company's market capitalization to EUR 98.55 billion — though no direct causal link between the FDA designation and that advance has been established. Since the MiniMed spinoff was announced roughly a week ago, the stock has added 2.2%, while MiniMed shares themselves have risen 3.0% over the same stretch.
The coming weeks present a clear set of markers. Whether the exchange offer draws sufficient participation by October 9 will shape how cleanly Medtronic can sharpen its focus on higher-margin core therapies. Meanwhile, the IN.PACT BTK pivotal study — with its 400-patient target — becomes the operational yardstick for the quarters ahead. If enrollment proceeds on schedule and endpoints prove statistically significant, Medtronic opens a below-the-knee treatment segment where interventional options remain scarce. Delays or disappointing efficacy data, by contrast, would quickly erode the regulatory goodwill the FDA has extended.
