Fatal level crossing crash: Warning systems failed before train struck car, RAIB finds

A fatal collision at a level crossing in Lancashire last month occurred after automated safety systems failed to activate, according to preliminary findings from the Rail Accident Investigation Branch (RAIB).
The crash at Hoghton level crossing on June 25, 2026, killed 33-year-old Kerry Coombes and left her eight-year-old daughter in critical condition. The RAIB's preliminary report, released on July 16, found that the crossing's traffic lights did not flash, audible alarms did not sound, and the half-barriers remained raised as a Northern service train approached from Colne to Preston.
The train had passed a green signal before striking the car at approximately 08:50 BST, the investigation established.
Criminal investigation underway
While the RAIB focuses on the technical failures of the signalling and barrier systems, a separate criminal investigation is also ongoing. Two railway staff members were arrested and subsequently de-arrested in the early stages of the inquiry.
The multi-agency response reflects the severity of the incident and the questions it raises about safety at level crossings across the UK network.
Network Rail apologises
Network Rail has confirmed it is cooperating fully with the RAIB and other authorities. The organisation issued apologies following the disclosure that the crossing's protective measures had not functioned.
The RAIB's full investigation will continue to examine why the warnings failed to activate despite the train receiving a proceed signal — a critical safety question for the rail industry and the thousands of motorists who use level crossings daily.
Public calls for closure
Local concern over safety at the site has grown sharply since the crash. A public petition calling for the permanent closure of Hoghton level crossing has surpassed 1,000 signatures, reflecting community anger and anxiety about the crossing's reliability.
