- Management narrowed the 2026 operating-income outlook to flat to a negative mid-single-digit percentage change at constant currency and excluding special items.

Fresenius Medical Care’s third-quarter update leaves investors weighing operational progress against a more cautious profit path. In its November 3, 2026 press release, the company said it had narrowed its 2026 operating-income outlook to a range from flat growth to a negative mid-single-digit percentage change, measured at constant currency and excluding special items.
The release from Fresenius Medical Care said revenue growth is still expected to be broadly flat for 2026. Management cited operating-income development during the first nine months and anticipated fourth-quarter headwinds as the reasons for moving to the lower half of the previous range.
Technology rollout supports the core business
The U.S. rollout of the 5008X CAREsystem reached 348 clinics by September 30, representing 16 percent of the company’s U.S. dialysis machines. Treatments performed with the system reached 1.3 million, supporting the company’s effort to expand its kidney-care offering.
Underlying performance in Care Delivery also improved. Operating income excluding special items rose 4 percent in the quarter, while the segment margin increased to 14.7 percent. The company said the improvement reflected positive rate effects and FME25+ savings that offset reimbursement and labor-cost pressure.
Reported profit absorbs China costs
Group revenue rose 1 percent year over year to 4,937 million euros in the third quarter, while reported operating income fell 19 percent to 384 million euros. Net income attributable to shareholders declined 42 percent to 160 million euros, mainly because the refined China strategy generated 108 million euros in one-time costs.
Excluding special items, operating income fell 5 percent to 545 million euros and the margin was 11.0 percent. That distinction matters for investors because the narrowed outlook excludes special items, while reported earnings continue to reflect restructuring effects.
