- Management cited stronger underlying performance in Care Delivery and continued progress with the 5008X CAREsystem rollout.
Fresenius Medical Care narrowed its 2026 operating-income outlook after reporting third-quarter results on November 3. Revenue was broadly stable, but regulatory effects, inflation and one-time costs related to the companys China strategy weighed on reported earnings.
The company said in its press release, carried by FinanzNachrichten, that third-quarter revenue rose 1% year on year to 4,937 million euros. At constant currency, revenue declined 1%, while organic growth was 0.1%.
Profit pressure limits full-year view
Reported operating income fell 19% to 384 million euros from 477 million euros a year earlier. The operating margin declined to 7.8% from 9.8%. Net income attributable to shareholders dropped 42% to 160 million euros, and basic earnings per share fell to 0.61 euros from 0.94 euros.
Adjusted operating income, which excludes special items, declined 5% to 545 million euros, or 6% at constant currency. The adjusted margin was 11.0%, compared with 11.7% in the prior-year quarter. Fresenius Medical Care said special items included costs from its China strategy refinement and the FME25+ transformation program.
The China measures included the discontinuation of local production and sales of the 4008A hemodialysis system and the exit from the peritoneal dialysis business. The resulting one-time costs amounted to 108 million euros, primarily from impairment, scrappage and termination costs.
Management points to underlying gains
Management said the quarter reflected continued underlying improvement despite the expected negative effects of TDAPA reimbursement regulations and higher inflation. Care Delivery operating income increased 18% to 497 million euros, while adjusted operating income in the segment rose 4% to 513 million euros.
Underlying operating income in Care Delivery, excluding negative TDAPA effects, improved 21% year on year. The company also reported that its 5008X CAREsystem was available in 348 US clinics by September 30, representing 16% of dialysis machines in its US clinics. The system had been used for 1.3 million treatments.
For the full year, Fresenius Medical Care continues to expect broadly flat revenue growth at constant currency. It now expects adjusted operating-income growth to range from flat to a negative mid-single-digit percentage, compared with the previous range that extended from a mid-single-digit decline to a mid-single-digit increase. The revised range is based on the first nine months and expected fourth-quarter headwinds.
