Fresenius Medical Care reported third-quarter results on Tuesday, November 3, 2026, showing stable underlying revenue but continued pressure on profitability. In its quarterly release, the kidney-care group said operating income excluding special items fell 5 percent year over year to 545 million euros, or 6 percent at constant currency.
Reported operating income declined 19 percent to 384 million euros. Reported net income attributable to shareholders fell 42 percent to 160 million euros, largely because of one-time costs linked to the refinement of the company's China strategy. Adjusted net income declined 10 percent to 291 million euros, while adjusted basic earnings per share remained stable at 1.11 euros.
Revenue holds as profit pressure persists
Group revenue increased 1 percent to 4.937 billion euros in the quarter, although it declined 1 percent at constant currency and grew 0.1 percent organically. Care Delivery revenue rose 3 percent to 3.492 billion euros, while Care Enablement revenue increased 7 percent to 1.451 billion euros. Value-Based Care revenue fell 7 percent to 537 million euros after a change in the risk structure of one contract.
Management pointed to regulatory and inflationary pressure, particularly the effect of TDAPA reimbursement rules and higher labor costs. Care Delivery adjusted operating income increased 4 percent to 513 million euros, while Care Enablement adjusted operating income rose 3 percent to 106 million euros despite the broader cost pressure.
U.S. technology rollout advances
Fresenius Medical Care said its 5008X CAREsystem was available in 348 U.S. clinics by September 30, representing 16 percent of its U.S. dialysis-machine base. The company said 1.3 million treatments had been performed with the system and kept its target of converting about 20 percent of the U.S. installed base by year-end.
The company also launched the multiFiltratePRO system in the United States in October for critically ill patients with acute kidney injury. In China, it discontinued local production and sales of the 4008A system and exited its peritoneal dialysis business while maintaining existing obligations. The related one-time costs totaled 108 million euros.
Outlook moves toward lower range
Fresenius Medical Care maintained its expectation for broadly flat 2026 revenue compared with the prior year. It narrowed its adjusted operating-income outlook at constant currency to a range between flat growth and a negative mid-single-digit percentage change, citing first-nine-month performance and expected fourth-quarter headwinds.
The FME25+ transformation program generated 74 million euros of additional sustainable savings in the quarter. The company expects 250 million euros of sustainable savings for 2026 and said the program remains a central part of its effort to offset inflation and reimbursement pressure.
