Outlook rises as margins matter

  • Elevance Health lifted its 2026 adjusted earnings outlook after stronger third-quarter operating performance.
  • Medicaid, Medicare Advantage, membership and health-plan costs remain central tests for the 2027 earnings path.
  • The company expects at least 12 percent adjusted earnings-per-share growth in 2027 from a higher earnings baseline.

Elevance Health raised its full-year 2026 adjusted earnings outlook after reporting stronger-than-expected third-quarter operating performance on October 21. In its earnings release, the insurer said adjusted diluted earnings per share reached USD 6.99 and operating revenue rose 0.7 percent year over year to USD 50.4 billion.

Management raised 2026 adjusted diluted EPS guidance to at least USD 29.00 and lifted operating cash flow guidance to at least USD 6.5 billion. The company also set a higher earnings baseline of at least USD 26.75 for 2026, excluding non-recurring investment income, other non-operating items and targeted one-time investments.

Operating gains support higher outlook

Elevance said the quarter benefited from higher premium yields, increased CarelonRx product revenue and growth in service fees. Those gains partly offset lower Medicare Advantage, Medicaid and employer group risk membership.

The benefit expense ratio improved by 70 basis points to 90.6 percent, helped by better performance in Medicare Advantage and Individual ACA. At the same time, the adjusted operating expense ratio increased to 11.3 percent as the company invested in capabilities intended to support longer-term growth.

The Health Benefits segment generated USD 43.4 billion in third-quarter operating revenue, up 3 percent from the prior-year quarter. Carelon revenue increased 5 percent to USD 19.3 billion, while Carelon operating gain rose 17 percent to USD 0.9 billion. Medical membership stood at 44.9 million at September 30, down 487,000 from a year earlier.

2027 growth depends on execution

Chief Executive Gail K. Boudreaux said the company remains confident in delivering at least 12 percent adjusted diluted EPS growth in 2027 from the higher baseline. The outlook places continued weight on Medicaid reimbursement, medical-cost control, Medicare Advantage performance and the development of Individual ACA business.

Elevance Health stock traded at EUR 385.45 on Lang & Schwarz at 14:00 CET on Friday, November 6, 2026.

Investors will also track the quarterly dividend of USD 1.72 per share, payable on December 18 to shareholders of record at the close of business on December 4. The next earnings update will show whether the improved benefit expense trend and Carelon momentum are translating into sustainable operating gains.

US health insurers remain sensitive to policy expectations, medical-cost trends and government-sponsored plan economics. Current market reports provide the broader S&P 500 context.
